Breast cancer metastases - cervical spine

Diagnosis almost certain

Presentation

Right ulnar-distribution weakness.

Patient Data

Age: 60 years
Gender: Female

Loss of normal T1 marrow signal within C5, as well as C7 to T3 vertebral bodies extending into the posterior elements of C7 and T1 with associated bone marrow edema and vivid post-contrast enhancement.

Enhancing extra-osseous soft tissue extension at the level of C7/T1 with obliteration of the normal epidural and neural exit foraminal fat signal with a similar appearance at T1/2 and abnormal epidural enhancement to the level of T3.

 

Case Discussion

This patient had a history of breast cancer, which was treated a few years ago with chemotherapy and radiation therapy. The MRI changes almost certainly represent breast cancer metastases.
