Breast hamartoma (tomosynthesis)

Presentation

Right breast palpable mass on physical examination. On ultrasonography evaluation, a heterogeneous, lobulated mass was reported in the right breast.

2D and tomosynthesis

A heterogeneous density mass with fat and soft tissue density components and lobulated margin that is better seen on tomosynthesis images is noted in the lower outer quadrant of the right breast. No internal calcification or surrounding parenchymal distortion could be identified.

Screening mammography one year later shows no changes in the appearance and size of the right breast mass.

Case Discussion

A fat containing, lobulated mass in the right breast of a 55-year-old patient, presenting with a palpable mass on physical examination, is compatible with a BI-RADS 2 lesion and consistent with breast hamartoma, a benign breast lesion. 

As seen in this case, in dense breasts with obscured masses, digital breast tomosynthesis could be more helpful for better evaluation of the breast lesions, particularly the margin and internal components.
