Presentation
Right breast palpable mass on physical exam.
Patient Data
A large hetero-dense mass with an irregular and spiculated margin is noted in the UOQ of the right breast that causes surrounding parenchymal distortion. In addition, a rounded, slightly lobulated mass is observed posterosuperior to the mentioned mass. (BI-RADS 5)
A part of a large lymphadenopathy is also evident in the right axillary regions in the MLO view.
Mild skin thickening is present in the peri-areolar regions of the right breast.
The patient underwent an ultrasound-guided core needle biopsy.
Pathology:
Nests of tumoral cells float within mucinous pools, and abundant mucin is around tumoral cells. Fibrous septa separate mucin pools. Tumoral cells have a low nuclear grade and small eosinophilic cytoplasm.
Diagnosis: Breast mucinous carcinoma
Case Discussion
Mucinous carcinoma of the breast is a subtype of invasive ductal carcinoma that tends to occur in older women. On mammography, the presence of mucin results in a low-density and relatively well-defined lobular mass. Calcifications are rare in pure mucinous types.
