Brucella spondylodiscitis - pediatric

Diagnosis almost certain

Presentation

Few weeks history of low back pain with fever.

Patient Data

Age: 12 years
Gender: Female

Narrowing of the L3-L4 disc space. The L3 and L4 vertebral bodies,show a low signal on T1, high signal on T2 and STIR with enhancement on post-contrast sequences (bone marrow edema). Thickening with enhancement of the prevertebral space mainly on the left with small abscesses formation as well as edematous infiltration of the left psoas muscle.

Moderate hepatosplenomegaly with no focal hepatic or splenic lesion.

Case Discussion

The MRI features are those of L3-L4 spondylodiscitis. The laboratory investigations were positive for Brucellosis
