Twisting injury.

Patient Data

Age: 30 years
Gender: Male

The MRI sequences demonstrate:

  • absent bow tie sign  of the medial meniscus on the sagittal sequences

  • double PCL sign on the sagittal sequences: the flipped medial fragment lies anteroinferior to the PCL mimicking a second smaller ligament

  • double anterior horn/flipped meniscus on sagittal sequences

  • double delta sign on sagittal sequences

  • meniscal fragment in the intercondylar notch on coronal sequences

  • moderate joint effusion

  • intact ACL and collateral ligaments

Bucket-handle meniscal tears are a type of displaced vertical meniscal tear where the inner fragment is displaced centrally. They more commonly occur in the medial meniscus and are often associated with anterior cruciate ligament (ACL) tears.
