Bike accident and twisting of knee

Patient Data

Age: 25 years
Gender: Male
mri

Extensive bony marrow contusion involving lateral tibial and femoral condyles, to a lesser extent medial tibial and femoral condyles and the fibular head. Reduced volume of posterior horn of lateral meniscus that is flipped anteriorly. Lax but intact anterior cruciate ligament which appears contused. Moderate retropatellar effusion.

Case Discussion

Double PCL sign and flipped meniscus sign are positive in this case of acute knee twisting injury. This is a classic example of bucket handle meniscal tear. A not so typical double PCL appearance looks like a fragment in the intercondylar notch
