Longitudinal tear involving the anterior horn, body, and posterior horn of the medial meniscus, with a displaced meniscal fragment in the intercondylar notch in keeping with bucket handle tear. The tear is giving the double PCL sign on sagittal images.
Fluid at the site of the ACL femoral attachment on coronal PD fat sat images in keeping with a complete tear of the ACL, and giving the empty notch sign.
Anterior translation of the tibia relative to the femur on sagittal images (anterior tibial translocation sign).
Mild joint effusion.

Case of bucket handle medial meniscal tear, associated with a complete ACL tear.
The bucket handle tears are much more common in the medial meniscus, and are often associated with an anterior cruciate ligament tear.
