Bucket handle tear of medial meniscus

Presentation

Twisting injury of knee.

Patient Data

Age: 40 years
Gender: Male

High-grade tear of mid-segment and tibial attachment of anterior cruciate ligament.

A complex tear of posterior horn and body of medial meniscus is seen, possibly extending into anterior horn with a large displaced meniscal fragment, measuring approx. 32 x 07 mm in intercondylar notch, aligned anterior to posterior cruciate ligament.

Near full-thickness cartilage defect, measuring approx. 9 mm is seen along medial femoral condyle with subchondral marrow edema.  

Low-grade sprain of medial collateral ligament almost along the entire length as depicted by edema superficial to the ligament. Continuity of the ligament is as such maintained.

Low-grade sprain of femoral attachment of lateral collateral ligament and popliteus tendon is seen. 

Low-grade sprain of mid-segment and patellar attachment of medial and lateral patellofemoral ligaments seen.

Low-grade sprain of semimembranosus tendon and posteromedial joint capsule seen. 

Lobulated altered signal intensity lesion, measuring approx. 20 x 10 mm is seen in distal shaft of femur – likely benign chondroid lesion, possibly enchondroma (likely incidental finding).

Mild strain of medial head of gastrocnemius muscle is seen.

Moderate joint effusion is seen with edema/fluid along semimembranosus and medial head of gastrocnemius muscles.

Significant edema is seen in infrapatellar fat pad.

Lateral meniscus and posterior cruciate ligament are unremarkable.

Case of anterior cruciate ligament tear with complex tear of posterior horn and body of medial meniscus, possibly extending into anterior horn, a large displaced meniscal fragment in intercondylar notch, aligned anterior to posterior cruciate ligament consistent with Bucket handle tear of medial meniscus.
