Burkitt lymphoma of the orbit: pre and post chemotherapy

Presentation

This young boy with histologically proven Burkitt lymphoma. Presented initially with pain and proptosis of the left eye and headaches for a 2-month duration.

Age: 5
Gender: Male

This baseline study demonstrates the extent of the multiple tumor prior to chemotherapy in the base of skull extending into the sphenoid bone and superiorly in the lateral aspect of the left globe (extraconal), inseparable from the lateral rectus muscle. This is causing severe proptosis of the left globe. In addition, there is another calvarial mass lesion in the right occipital region, which extends internally and seems to breach the dura.

This study was done at exactly 2 months after the initial study demonstrated complete resolution of all the masses that were seen on the initial scan.

Case Discussion

Burkitt lymphoma is a type of non-Hodgkin lymphoma, more common in the so-called "Burkitt lymphoma belt" where there is a high prevalence of malaria. It has also been associated with Epstein-Barr virus. This type of lymphoma responds very well to chemotherapy alone (Cyclophosphamide, Doxorubicin, Vincristine, Methotrexate and prednisolone in this case)

This case demonstrated the good response of Burkitt lymphoma to chemotherapy on imaging.
