Long standing ulcer in the left buttock. Now fever and left hip pain. Past history of spina bifida

Gas in the soft tissues with sclerosis and cortical irregularity of the left ischial tuberosity.

Typical spina bifida defect in the lumbo-sacral spine with a ulcer defect in the left buttock extending down to the ischial tuberosity with sclerosis and cortical thickening (chronic osteomyelitis) together with subtle irregular cortical erosion (acute osteomyelitis).  Note surrounding soft tissue swelling.
