C2-3 spondylolisthesis

Case contributed by Doaa Faris Jabaz
Diagnosis almost certain

Presentation

An incidental finding while evaluating a long-standing painless neck mass.

Patient Data

Age: 55 years
ct

Grade II C2-3 spondylolisthesis with bilateral well-corticated defects at mid-portion of C2 lamina (a gap of 5mm with several interposed bone fragments), no other vertebral dysplasia to strongly suggest developmental cause thus repetitive microtrauma and post-traumatic disjunction can be considered, no feature of atlantooccipital dissociation.

Right-sided 2nd branchial cleft cyst/ Bailey classification type II.

Case Discussion

Grade II C2-3 spondylolisthesis related to C2 spondylolysis (the patient provided a history of a car accident when he was 15-20 years old and treated non surgically thus post-traumatic disjunction is likely).
