Small subtle non displaced fissure fracture of persistent dark signal intensity is seen traversing the anterior beak of the calcaneus bone with surrounding extensive marrow edema signal eliciting high signal at STIR WI.

Case Discussion
Anterior process calcaneal fracture.
Accessory ossicle (os calcaneus secundarius) can be difficult to distinguish from an anterior process fracture, especially if the fracture is subacute or chronic and lacking the surrounding marrow edema as in this case.
