Calcaneal fractures with associated L2 burst fracture

Presentation

Fall from height onto feet.

Patient Data

Age: 30 years
Gender: Male

CT shows a comminuted fracture of both calcanea. 

In addition, there is a comminuted fracture of navicular, cuboid and cuneiform lateral bones of the left foot.

A compression burst fracture of the L2 vertebral body. The fracture involves the posterior vertebral body cortex. Retropulsion of a bony fragment into the spinal canal up to 8 mm.

In addition, there is a fracture of the left transverse process of L3.

Case Discussion

In the case of an axial loading trauma mechanism and a fractured calcaneus, the associated lumbar spine injury needs to be ruled out. 

Typical findings of burst fracture seen in this case are: 

  • involving the posterior vertebral body cortex
  • retropulsion of bone fragment
