Calcaneal osteomyelitis

Presentation

Opening into the foot discharging pus. Twenty years of history of diabetes mellitus.

Patient Data

Age: 55 years
Gender: Female
mri

Extensive bone marrow signal changes (T1 hypointense, T2 and PD hyperintense) involving the calcaneus compatible with osteomyelitis. 

A sinus tract starts from the calcaneus base into the skin on the plantar aspect medial side.

Abnormal bone marrow signal isointense on T1 and hyperintense on T2/PD FS images involving the midfoot bones is suggestive of osteitis.

Extensive edema and fluid signal intensity also are seen within the midfoot and hindfoot intrinsic muscles and subcutaneous tissues.

Achilles tendinopathy and plantar fascia calcaneus insertion tear also are the other findings.
