MRI demonstrating abnormal midline structures including:
- hypoplasia or dysgenesis of the posterior two thirds of the corpus callosum
- posterior pituitary signal located in the hypothalamus
- small optic chiasm
- large arachnoid cyst in the left middle cranial fossa
- bilateral periventricular nodular heterotopias more marked in the right
- incomplete fornix on the right
- communication between the frontal horn of the right lateral ventricle and the interhemispheric CSF

This case nicely illustrates how multiple congenital malformations often co-exist.
