Diagnosis almost certain

Presentation

Back pain in a patient with a history of neglected spinal trauma a few years ago.

Patient Data

Age: 65 years
Gender: Female
mri

The MRI sequences demonstrate a moderate thoracolumbar kyphosis centered on T12 vertebral body which shows an old compression fracture. Diffuse and extensive atrophy with fatty replacement of the paraspinal muscles.

No disc herniation is seen.

Normal dimensions of the spinal canal.

Case Discussion

Camptocormia (bent spine syndrome or cyphose hystérique) is a rare syndrome characterized by involuntary flexion of the thoracolumbar spine with weight-bearing which reduces when laying down. This condition may be seen in various diseases such as Parkinson/Alzheimer diseases, dystonic/neuromuscular disorders, as well as following spinal trauma with vertebral fracture.
