Diagnosis almost certain

Presentation

Right proptosis since birth.

Patient Data

Age: 2 months
Gender: Female

Orbits

mri

Right orbital extra-conal soft tissue lesion, mainly at the lateral and superior orbital regions. It is measuring 2.7 x 1.3 x 2.8 cm in AP, SS and CC dimensions. It shows homogeneous texture with low T1 and high T2, no diffusion restriction, no fat suppression, with intense post-contrast enhancement.

Ir exerts mass effect in the form of mild proptosis, compression of retrobulbar fat and extra-ocular muscles and scalloping of lateral orbital wall. An anterior extraconal extension is noted.

Case Discussion

MRI imaging features and patient's age are suggestive of capillary hemangioma of the orbit.

Capillary hemangioma of the orbit is different from orbital cavernous hemangioma. It is usually present since birth anterior to the globe with imaging characteristics of avid homogenous enhancement in contrast-enhanced CT.
