By looking at the MLO view you may initially think this is a small intramammary lymph node. The position is acceptable. But on the CC view its unusually medial in the breast. And the teaching point is its too dense for a node.

A small infiltrating ductal carcinoma.
Case Discussion
Any post-menopausal woman who develops a change in breast imaging deserves careful thought.
The lesion is too dense to be discounted as a lymph node. Not all lesions projected over the pectoral profile on the MLO view are lymph nodes.
