Case contributed by Naqibullah Foladi
Diagnosis almost certain

Presentation

Neck bulging

Patient Data

Age: 45 years old
Gender: Male
ct

Intensely enhancing mass lesion in the bifurcation of the right common carotid artery.
Significant splaying of the internal and external carotid arteries.
External and internal jugular veins seem enhancing and slightly prominent.

Case Discussion

The imaging characteristics are compatible with carotid body tumor (Paraganglioma).

The possible differential is vagal schwannoma, but that would spread internal and external jugular veins and push the internal carotid artery anteromedially.

The other differential is jugular paraganglioma, but it would be located more rostrally.
