Presentation

Patient presents with red eye in the right side 2 months ago.

Patient Data

Age: 70 years
Gender: Female

The CT scan demonstrates abnormal dilation in the right superior ophthalmic vein and the left cortical vein.

An indirect carotid venous fistula was confirmed by a DSA examination. The feeding arteries are the bilateral ascending pharyngeal artery and middle meningeal artery. The fistula drains into both the right superior ophthalmic vein and the left cortical vein.

Case Discussion

Based on its characteristics, it was classified as Barrow classification of caroticocavernous fistulae Type C and Cognard classification of dural arteriovenous fistulas Type III.
DSA remains the gold standard for diagnosing carotid cavenous fistula and was a suitable choice for treating this particular case. The patient underwent Onyx embolization, which successfully resolved patient's red eye.
