Chronic sinusitis and rhinorrhea. Long term use of nasal decongestants. Nasal septum perforation on examination

CT scan to assess the paranasal sinuses

Small hole in the central cartilaginous nasal septum. Bony septum intact.

Case Discussion

Vasoconstrictors in the nasal decongestants cause local ischemia/infarction of the cartilage with chronic use in much the same way as cocaine in illicit users.
