Presentation

Fall from height.

Patient Data

Age: 92-year-old
Gender: Female

CT Brain

No acute brain abnormalities. Right frontal subgaleal hematoma. Dense right lens. 

Ultrasound right orbit

Fall with right periorbital bruising. Has a dense cataract precluding the assessment of the retina.

The lens shows increased and heterogeneous echogenicity in keeping with a cataract. Vitreous body has normal sonographic appearances, with homogeneous content and with no features of hemorrhage. No evidence of retinal detachment. The medial scleral calcification demonstrated on CT was not identified sonographically.

Case Discussion

This case illustrates a right sided cataract assessed with ultrasound due to suspicion of posterior globe pathology. The opaque lens precluded visualization of the back of the eye in the ophthalmologic examination.
