Cecal bascule

Case contributed by Matt A. Morgan
Diagnosis certain

Presentation

History of metastatic colon cancer status post left hemicolectomy and hepatic segmentectomies/metastatectomies. He presents with acute abdominal pain.

Patient Data

Age: 70 years
Gender: Male
ct

There is a dilated cecum with dilated upstream small bowel with a short segment narrowing in the ascending colon, just above the level of the ileocecal valve. The colon downstream from the narrowing is collapsed.

Although there is a mild twist at the level of the narrowing, the cecum appears to be more "flipped up" into the midbdomen rather than there being a "twist" in the mesenteries supplying the cecum.

Case Discussion

The patient went to the operating room shortly afterward and a cecal bascule (a type of cecal volvulus) was confirmed.
