Medial wrist pain following a high speed motor bike accident
Patient Data
Central fracture dislocation of the metacarpo-hamate joint due to the 5th metacarpal base (arrow) being impacted onto the central hamate. In addition there is a comminuted fracture at the base of the 4th metacarpal due to imoaction with the capitate.
Case Discussion
Unusual injury due to impaction of a clenched wrist against a hard surface causing the hamate to be the cleeved in coronal plane by the 5th metacarpal base. Typically these fracture dislocations are associated with a further impaction injury/fracture to the 4th metacarpal base (against the distal capitate). The more common hamate fracture involves an avulsion of the hook occurring in professional golfers and racquet sports.
