Presentation

Headache, recurrent vomiting and imbalance.

Patient Data

Age: 25 years old
Gender: Male

  • Mostly cystic partly soft tissue mass within the right cerebellar hemisphere.
  • Significant mass effect on the brainstem and the fourth ventricle which appears of abnormal shape.
mri
  • Intra-axial cystic right cerebellar hemispheric mass lesion containing intensely enhancing peripheral mural nodule. The cystic part is isointense T1, hyperintense T2/FLAIR and the soft tissue nodule is isointense on T1 and T2.
  • The mass effect is better appreciated by MRI; compression effect on the brainstem and the fourth ventricle, crowded foramen magnum.

Case Discussion

The differential diagnosis for a posterior fossa cystic mass with a mural nodule in an adult includes hemangioblastoma, pilocytic astrocytoma and metastasis, among others.

Following resection of this mass, the histology was that of a hemangioblastoma.
