Presentation
Sudden onset of headache with nausea and vomiting.
Patient Data
Posterior fossa haemorrhage involving the right cerebellar hemisphere. It extends medially, involving the 4th ventricle. Intraventricular blood noted within the 4th, 3rd and right lateral ventricle
Mild cerebellar tonsillar herniation noted at the foramen magnum with compression of the brainstem due to the mass effect of the haemorrhage.
Dilatation of temporal horn of lateral ventricles, representing early hydrocephalus
Case Discussion
A case of an elderly patient with hypertension presenting with large cerebellar haemorrhage with early tonsillar herniation.
Given the rapidity of the bleed and the comorbidities, the patient received palliative care.
