A large left cerebellopontine angle mass lesion with broad dural base, expressing homogeneous low signal intensity on T1 and T2, homogeneous post-contrast enhancement and no restricted diffusion. It is extending through the left internal auditory canal. It is indenting the left side of the pons.

Case Discussion

MRI appearances favor left cerebellopontine meningioma rather than schwannoma, for the following reasons:

  • meningioma has a low T2 signal, schwannoma has a high T2 signal
  • both can extend through internal auditory canal, schwannoma is more known for this as it begins inside the canal
  • it is hard for that sized schwannoma not to have cystic changes, which are not present
  • wide dural base
