The left cerebral hemisphere shows:

  • thinning of the grey matter cortex
  • reduced volume of the underlying white matter mainly affects the frontoparietal region
  • enlargement of the ipsilateral lateral ventricle as well as ipsilateral cisterns
  • encephalomalacia involving the left high parietal region
  • reduced size of the left (ipsilateral) cerebral peduncle
  • As well as reduced size of the contralateral cerebellum

Note small-sized contralateral (right) cerebellar hemisphere.

This is a case of cerebral hemiatrophy, which has many different etiologies, congenital and acquired.
