Diagnosis probable

Presentation

Long standing headaches.

Patient Data

Age: 55 years
Gender: Male
mri

Homogeneously enhancing showing diffusion restriction subependymal lesions are seen along frontal horns, posterior bodies of both lateral ventricles, involving septum pellucidum, the body of corpus callosum, genu of the corpus callosum on the left side and reaching up to superomedial aspects of both thalami.

Similar lesions are seen along the area postrema, within the inferior aspect of the fourth ventricle possibly involving the adjacent vermis, left cerebellar hemisphere, and within the third ventricle, indenting the left thalamus and superior aspect of the midbrain.

A similar lesion is seen in the right basifrontal lobe.

The perfusion images (CBV and CBF) show increased perfusion seen as areas of green, yellow and red.

Case Discussion

The imaging features suggest a possibility of cerebral lymphoma.
