Diagnosis almost certain

Presentation

Left-sided weakness for 10 days.

Patient Data

Age: 55 years
Gender: Male

Brain

mri

Right frontoparietal linear areas of FLAIR hyperintensities that showed restriction on DWI, with no enhancement after contrast administration.

Engorged right frontoparietal cortical vessels, noted on SWI.

Abnormal signal noted within the superior sagittal sinus.

Thrombosis of the right sigmoid, transverse sinus as well as the entire superior sagittal sinus.

Case Discussion

Right-sided deep venous sinus and superficial venous cortical thrombosis with areas of restriction and blooming along the frontoparietal cortex suggesting venous hemorrhagic ischemia.
