Cerebral venous thrombosis (CVT)

Diagnosis almost certain

Presentation

Sudden onset of headache with bilateral papilledema in a diabetic hypertensive patient.

Patient Data

Age: 45 years
Gender: Male

Thrombosis of the superior sagittal sinus and right lateral and sigmoid sinuses extending to the ipsilateral internal jugular vein is seen as an intermediate to high signal on T1 and FLAIR (sigmoid sinus, transverse sinus) and high signal on T2 with negative enhancement on the postcontrast sequences and loss of their signal on MRV 2D-TOF.

Moderately dilated cortical parietal veins bilaterally of high signal on T1, FLAIR and DWI and low signal on T2* in keeping with cortical vein thrombosis.

Small vessel ischemic changes are noted.
