Normal craniocervical junction. Loss of the normal cervical lordosis.

Small left paracentral C5/C6 disc bulge causing mild/moderate narrowing of the left foraminal ostium.

Large left paracentral C6/C7 disc extrusion filling the left side of the cervical canal, occluding the left foraminal ostium and indenting the ventral aspect of the cord. No myelomalacia.

The remaining disc levels are normal.

Case Discussion

This is a case of a large C6/C7 disc extrusion impinging on the exiting left C7 nerve root, which is entirely consistent with the clinical suspicion. The triceps receives its innervation from C7, hence the wasting of this muscle on clinical examination.

Radiology is always fun when good clinical information is provided and one can correlate the imaging findings.
