Cervical extramedullary mass - histology unknown

Diagnosis probable

Presentation

Complaining of both upper and lower limb weakness.

Patient Data

Age: 25 years
Gender: Male

There is well defined extra medullary hyperintense signal intensity lesion seen on the T2W images in the spinal canal from cervico-medullary junction to C2 vertebral level on left side. The lesion is seen involving the spinal canal, left C1-2 neural foramina and extending outside the neural foramina at left paravertebral region. The lesion causes compression and right lateral displacement over the cervical spinal cord. There is enlargement of the left C1-2 neural foramina. The lesion measures approx. 3.0 (SI) x 2.1 (AP) x 4.0 (TR) cm in size.

 Hyperintense signal is seen on the T2W images in the spinal cord at C2 vertebral level, may represent cord edema/gliosis.

Small hyperintense signal lesion seen on the STIR images in the right paravertebral region adjacent to T1-2, T2-3 neural foramina. 

Case Discussion

These findings represent extramedullary neoplastic lesions like nerve sheath tumor (e.g. neurofibroma/schwannoma).
