Cervical pseudosubluxation

Presentation

Neck pain post fall.

Patient Data

Age: 4 years
Gender: Male
x-ray

Neck held in flexion. Anterolisthesis of C3 on C3 and to a less extent C3 on C4. Facet joints are congruent. No prevertebral soft tissue swelling. 

Case Discussion

The main difficulty in these cases is differentiating pseudosubluxation from a true traumatic injury. A mild degree of anterolisthesis (which can be exacerbated by neck flexion) and the absence of prevertebral soft tissue swelling favors pseudosubluxation of a traumatic subluxation.
