Presentation

Abnormal uterine bleeding associated with pelvic pain. Has one previous cesarean scar.

Patient Data

Age: 40 years
Gender: Female
ultrasound

On the anterior wall of the uterus, below the cesarean section scar, there is a triangular hypoechoic collection of fluid. It is avascular on color Doppler imaging.

Within the cervix, there are a few Nabothian simple cysts.

Case Discussion

In this case, the presence of a uterine niche might explain the patient's abnormal uterine bleeding and pelvic pain. Uterine niches can develop as a result of an incomplete healing of the cesarean scar, resulting in a pouch-like indentation in the uterine wall. This may lead to stagnant menstrual blood within the niche, causing pain and irregular bleeding.
