Presentation
Right-sided pectoral region diffuse swelling for the last few weeks. Recent mild pain locally.
Patient Data
There is a well-defined, ovoid lesion in the right pectoral region. It measures about 100 (craniocaudal) x 90 (transverse) x 40 (anteroposterior) mm. The lesion is located between the pectoralis major and pectoralis minor muscles. Subclavian vessels are deep to the lesion. The lesion is hyperechoic to subcutaneous fat, hyperechoic to adjacent muscles along with the presence of linear incomplete internal striations present and a few thin septa. There are no calcification/ cystic changes. The lesion shows mild compressibility. Doppler shows few flow signals in the lesion.
Gross Description: multiple soft tissue pieces, whitish and brown, collectively measuring 7.6x7.4x4.4 cms, weighing 204 grams.
Microscopic Description: sections show features of angiolipoma.
Atypia or lipoblasts suggestive of liposarcoma are not seen.
No evidence of other malignancy.
Case Discussion
The case shows a chest wall lesion showing ultrasound features of a lipoma with few septa and flow signals. Surgical excision was done. Histopathology revealed the lesion being angiolipoma.
