Long term headache, vertigo, dysarthria

Patient Data

Age: 45 years
Gender: Female

Caudal descent of the cerebellar tonsils through the foramen magnum.

Small syrinx of the posterolateral medulla oblongata.

Cystic degeneration of the left cerebellar tonsil.

Intraoperative picture of the cystic degeneration of the left cerebellar tonsil.

Suboccipital decompression.

Subtotal regression of the medullar syrinx after subocciptal decompression.

Case Discussion

This case demonstrates a symptomatic Chiari I malformation (stage III) with brainstem compression and syrinx.

The patient was treated with decompression of the posterior fossa by removing part of the occipital bone and the posterior arch of C1.

The medullary syrinx showed a subtotal regression after the surgery.
