Chiari I malformation with hydrosyringomyelia

Peg-like caudal protrusion of the cerebellar tonsils into the upper cervical spinal canal with crowding and effacement of the CSF spaces at the level of the foramen magnum.
Downsloping of the tentorium is also detected.
There is focal cystic dilation in the center of the upper cervical spinal cord from the level of C2 (the syrinx is not visualized entirely).
Case Discussion
Chiari I is the most common amongst Chiari malformations.
Surgical decompression was indicated as patient is symptomatic and there is associated hydrosyringomyelia (syrinx). Completion of the cervical spine MRI is needed.
