Presentation
Neck pain for years. Numbness in hands.
Patient Data

14 x 9 x 98 mm (ML x AP x CC) T2 hyperintense, non enhancing ovoid region is seen within the spinal cord spanning from C2 to T1/2 level. This is most in keeping with a syrinx. Patchy T1 hyperintensity within the syrinx. This may represent proteinaceous material (most likely) as there is no gradient-echo change to suggest blood (not shown). No enhancement (not shown).
Pointed cerebellar tonsil extending 11 mm below the foramen magnum with obliteration of the cisterna magna.
Very minor central disc protrusions seen at the C3 to C7 levels. No canal or neural foraminal stenosis identified.
Case Discussion
This case demonstrates typical appearances of a Chiari 1 malformation with prominent cervical syrinx.
