Chiasmatic-hypothalamic glioma

Case contributed by Ammar Haouimi

Presentation

Visual disturbances.

Patient Data

Age: 70 years
Gender: Female

Lobulated suprasellar mass centered on the chiasma and hypothalamic region with extension to the anterior portion of the 3rd ventricle and interpeduncular cistern. It elicits an iso-to-low signal to the cortical grey matter on T1, inhomogeneous high signal FLAIR, and T2 with restricted diffusion and heterogeneous enhancement on postcontrast sequences. The optic chiasma is not visualized as a separate structure.

Moderate cerebral volume loss with small vessel ischemic changes.

Case Discussion

MRI features of a heterogeneously enhancing suprasellar mass centered on the optic chiasma and hypothalamus suggestive of chiasmatic-hypothalamic glioma.
