Presentation

Dyspnea, cough and fever.

Age: 90 years
Gender: Male
X-ray

Gas beneath the right hemidiaphragm, but bowel markings are evident.

Increased opacity of the right lung is appreciated.

Case Discussion

The chilaiditi sign refers to the usually asymptomatic interposition of the bowel (usually hepatic flexure of the colon) between the liver and the (right) hemidiaphragm. It is important to appropriately recognize it because it can mimick a pneumoperitoneum.
