Presentation

Abdominal cyst on antenatal US.

Patient Data

Age: 2 days
Gender: Female
ultrasound

Fusiform fusiform dilatation measuring 18 mm in maximum diameter, containing mobile sediment, possibly involving the intramural part of the CBD in the duodenum. Features favor choledochal cyst type Ia.
No intrahepatic bile duct dilatation.
Shrunken gallbladder.

8 days later

ultrasound

Follow-up ultrasound done 8 days later:

The cystic CBD is now filled with tumefactive sludge dotted with punctate crystals and has dilated further; it now measures 25 mm in maximum diameter and 30 mm in length.
No intrahepatic bile duct dilatation.

Case Discussion

Term neonate with neonatal jaundice. Hardly any prenatal screening.
"Cyst in center of abdomen" mentioned in the prenatal ultrasound report.

On postnatal ultrasonography, a choledochal cyst was demonstrated, compatible with Todani type I.

On follow-up US done 8 days later, the choledochal cyst had dilated further and had filled up with tumefactive sludge.
