Case contributed by Mohammad Taghi Niknejad
Diagnosis possible

Presentation

Abdominal pain.

Patient Data

Age: 70 years
Gender: Female
mri

The gallbladder is not seen at anatomical location due to prior resection.
Extra hepatic bile ducts are dilated and CBD measured 18mm in caliber with homogeneous internal signal intensity and normal distal tapering.
Two well defined high T2W signal lesions are noted at right lobe of the liver, measuring less than 12mm.

Case Discussion

Features may consistent with type Ia choledochal cyst. Although age-related and cholecystectomy-related biliary dilatation could be considered in differential list. No prior imaging was available for comparison.

Choledochal cysts represent congenital cystic dilatations of the biliary tree. Diagnosis relies on the exclusion of other conditions (e.g. tumor, gallstone, inflammation) as a cause of biliary duct dilatation.
