Case contributed by Anthony Nuñez
Diagnosis almost certain

Presentation

Patient presenting with a suspected stroke

Patient Data

Age: 75 years
Gender: Female

Multiple hypodense cystic lesions with peripheral calcifications along the choroid plexus located at the trigone of the lateral ventricles.

On MRI, the cystic lesions present isointense signal with CSF in T1 and T2, but the high signal in DWI consistent with bilateral choroid plexus xanthogranuloma.

Case Discussion

Choroid plexus xanthogranulomas are generally benign cystic lesions, bilateral and seen in elderly people. On MRI, the high signal in DWI is due to the mixture of lipid, fluid and blood products.
