Choroid plexus xanthogranulomas

Diagnosis almost certain

Presentation

Progressive cognitive impairment.

Patient Data

Age: 80 years
Gender: Female

There is diffuse cortical and subcortical atrophy. Bilaterally there are numerous T2 hyperintense white matter lesions, partially confluent, attributable to chronic small vessel ischemia.

In the lateral ventricles, there are multiple choroid lesions with high signal on T2 scans, isointense to CSF on FLAIR scans, high signal on DWI and low signal on ADC, with no discernible contrast uptake. This appearance is typical for choroid plexus xanthogranulomas.

Typical appearance of harmless choroid plexus xanthogranulomas, which can accumulate with age and should not be mistaken for pathologic intraventricular masses.
