Non-contrast CT of the brain demonstrates a small fluid density lesion near the right choroidal fissure without mass effect.

A small cystic lesion of CSF density is seen near to the choroidal fissure with a characteristic spindle or ovoid shape paralleling the long axis of the temporal lobe and choroid fissure on the sagittal images (blue arrow) measuring about 12 x 13 x 12 mm at AP, TV, and CC dimensions respectively.
Case Discussion
This case demonstrates an enlarged perivascular space mimicking a choroidal fissure cyst, both diagnoses are equally considered. It is a benign intracranial cyst, usually asymptomatic. No follow-up or any treatment is needed.
