Patient Data

Age: 60 years
Gender: Male

Cavernous transformation of the portal vein.

The portal vein is normal in caliber shows chronic thrombosis with partial recanalization at the periphery, the right and left branches show mild peripheral recanalization.

Free fluid in the right subphrenic space.

The superior mesenteric vein is patent as well as the splenic vein.

Splenomegaly. 

The hepatic artery is patent with a normal resistive index.

Patent hepatic veins and inferior vena cava.

The liver shows normal homogenous echo pattern, no signs of chronic liver disease. 

Status post cholecystectomy; however no-biliary dilatation.

Case Discussion

Portal vein thrombosis is an unusual finding in patients with sarcoidosis. This patient is known to have sarcoidosis 10 years ago and developed portal vein thrombosis. The liver is non-cirrhotic on ultrasound images and lab studies. Status post cholecystectomy 5 years ago.
