Chronic submandibular sialolithiasis

Case contributed by Tamer O. Abdo
Diagnosis almost certain

Presentation

Chronic right submandibular pain.

Patient Data

Age: 40 years
Gender: Male

Two well-defined calcified stones are seen medial to the right submandibular salivary gland, measuring 5 x 3 mm and 3 x 2 mm, mostly at the origin/proximal part of the submandibular gland duct.

The right submandibular gland appears smaller in size and of higher attenuation compared to the left side, with partial fatty replacement of its parenchyma. Appearances are consistent with chronic obstructive submandibular sialadenitis.

Case Discussion

Chronic obstruction of the submandibular gland duct causes atrophy and fatty replacement of the gland with prominence of the intraglandular ducts.
