Diagnosis probable

Presentation

Right-sided loin to groin pain for last 3 - 4 days.

Patient Data

Age: 55 years
Gender: Male
ultrasound

There is right-sided hydronephrosis and hydroureter. The proximal part of the right ureter is seen passing posterior to the inferior vena cava. It emerges to the right of the aorta, lying anterior to the right iliac vessels. There is a 5mm calculus in the right renal pelvis and another 5mm calculus in the distal third of the right ureter.

There is no calculus or back pressure in the left kidney. The urinary bladder is normal. Prostate shows mild enlargement.

Case Discussion

This adult male presented with typical right sided renal colic. Ultrasound showed obstruction at two levels. There is a right retrocaval ureter with a grossly dilated proximal ureter and moderate hydronephrosis. Ureter distal to IVC is mildly dilated due to distal third ureteric calculus.
