Presentation

Abdominal pain and elevated liver enzymes.

Patient Data

Age: 20 years
Gender: Female

The liver shows nodular outline and parenchyma with relatively hypertrophied caudate lobe, in keeping with cirrhosis. No CT-detectable hepatic hypervascular mass is noted in favor of HCC.

Dilated and engorged veins are seen especially at splenorenal and splenic hilar regions, features of portal hypertension. Portal, splenic and superior mesenteric veins are normally opacified. There is no evidence of cavernous transformation of the portal vein.

Huge splenomegaly is evident and its cranio-caudal height measured about 225 mm.
